This questionnaire is designed to assist in the evaluation of the health of your church. Its primary purpose is to give guidance for future direction and growth. The information supplied will be analyzed by Outreach Canada and presented to the church leadership. Your careful completion of each question (including those at the end) will be appreciated. Every effort possible is made to respect the confidentiality of your individual response. Nowhere in the questionnaire will you be asked to give your name or any information that would personally identify yourself.
